Could a Blood Test Help Spot Depression? (HealthDay)

FRIDAY, Feb. 3 (HealthDay News) -- Depression can be a tough condition to diagnose accurately, but new research suggests that someday a blood test might help.

It's not clear how much the test might cost, and it needs more stringent validation before it will be ready to be used in medical offices. Still, "it appears that these results are promising, after decades of research into finding a biological test for depression," said study author Dr. George Papakostas, an associate professor of psychiatry at Harvard Medical School.

The study was funded by the Ridge Diagnostics Co. and appeared in a recent issue of the journal Molecular Psychiatry.

It may seem like depression is an easy condition to diagnose and doesn't need a test to verify that it exists, but Papakostas said there are several ways that a blood-based depression test might be helpful.

For one, he said, a test could help doctors who aren't as experienced in psychiatric disorders. Also, he said, a test may provide assistance to doctors who aren't sure about the proper diagnosis of a patient: "This could be of help to them, in terms of guiding them in one way or another," he said.

Yet another use for a test would be to verify that a patient has depression, and therefore help him or her accept the diagnosis. "The majority of patients diagnosed with depression have no problem accepting the need for treatment," Papakostas said. However, "there is a minority of patients who feel that validation of an underlying process is helpful," he added.

In their study, Papakostas and his team gave a blood test to 36 patients with depression and 43 people who weren't depressed. The test looked for levels of nine different "biomarkers" in the blood that are associated with depression. These biomarkers are linked to inflammatory processes, the development and maintenance of brain cells, and interactions between brain structures associated with the stress response and other functions.

The researchers found that the test correctly identified patients with depression 91 percent of the time; the rest of the time it gave a false-negative diagnosis (it failed to spot the depression). The test correctly identified patients who weren't depressed about 81 percent of the time, giving false-positives the rest of the time.

The next step is to try to confirm these findings through further research, Papakostas said.

He didn't know how much the test might eventually cost, but he said it won't be as high as thousands of dollars and should be more akin to routine blood tests.

The test appears to detect inflammation in the brain, which has been linked to depression, Papakostas said. "That really doesn't surprise researchers. Chronic inflammation has been tied to a number of other illnesses in the kidneys, lungs and heart," he noted.

One outside expert said such a test would be welcome.

Dr. Michelle Riba, a professor of psychiatry at the University of Michigan who's familiar with the findings, said a blood test for depression could be helpful in several ways.

For one, it would be useful to identify people, especially children and adolescents, who are prone to depression and try to prevent it, she said.

Also, she said, a test could help give physicians insight into how depression treatments are working over time.

Accidental Kakapo Death Lowers Population of Rare, Flightless Parrots to 127 Birds

The death of an adult female kakapo (Strigops habroptila) on New Zealand?s Anchor Island this past weekend brings the population of these rare flightless parrots down to just 127 birds.

The late kakapo, known as Sandra, was killed when her transmitter harness got entangled in a tree. All kakapos are outfitted with transmitters to help rangers in the Kakapo Recovery program keep track of the birds. Sandra?s death marked the first time in 31 years of transmitter use that the devices have injured a bird.

?It?s gutting for the team to lose one of the birds this way,? Kakapo Recovery Program Manager Deidre Vercoe Scott said in a prepared statement. ?Without transmitters, our mission to support and grow the kakapo population would be virtually impossible.? She said the harnesses are designed and fitted so the birds can free themselves if they get caught, and the rangers are not sure why Sandra was unable to escape. Vercoe Scott told Radio New Zealand News that the recovery program will now review the use of the transmitter harnesses.

All kakapos live in semi-captivity on Anchor Island and Codfish Island, two predator-free havens off the coast of New Zealand. The birds?the world?s heaviest parrots and the only flightless parrot species?had no native predators until local Maoris brought dogs and rats to New Zealand 1,000 years ago. The arrival of European settlers in the mid-19th century hastened the birds? decline, and by the 1970s the species was thought to be nearly extinct in the wild. The only captive birds were all males. The discovery of a small population on New Zealand?s Stewart Island in 1977 inspired the creation of the recovery program that eventually relocated all known birds to predator-free islands. A very successful breeding program, which sometimes employs artificial insemination, brought the population to more than 100 birds for the first time in 2009.

Sandra?s death follows the January 2 death of a male kakapo named Waynebo. Although no clear cause of death was found, rangers estimate that Waynebo could have been more than 100 years old, and suggest he died of old age.

Two females named Monoa and Purity were found dead in September 2011, one on Codfish and the other on Anchor Island. Autopsies gave no obvious cause of death.

Eleven kakapo chicks were born last year, marking another successful breeding season. Unfortunately, there won?t be any hatchlings this year. Kakapos only breed when the fruit of the rimu tree is readily available, which only occurs every three years or so. This isn?t one of those years.

[Abstract]

Leukemia is a common malignant neoplasm of hematopoietic system. Its etiology and pathogenesis remain uncertain. Currently, chemotherapy is one of major approaches to treat it. However, the serious side effects of chemotherapy and MDR often lead to relapse of the disease and are eventually responsible for the failure of treatments. Thus, it is imperative to find out novel and more efficient antitumor drugs to improve the cure rate.In this study, we found out that linalool, a small-molecule compound, could inhibit the growth of various human lymphocyte leukemia cell lines significantly. Meanwhile, it showed little toxicity to normal hematopoietic cells. To determine effects of linalool on the growth of a serious of human leukemia cell lines and normal hematopoietic cells and to explore its possible mechanisms, we employed various technologies, including MTT assay, Flow cytometry, Human Cancer PathwayFinder II Gene Array, Western Blot, RT-PCR and so on.Materials and Methods1. Cell culture:?1? Human leukemia cell lines : H9. Molt-4 and Jurkat for T cell leukemia; Raji for Burkitt?s lymphoma; THP1 ? U937 for acute monocytic leukmia; K562 for chronic myelogenous leukemia?CML?; KG-1 for acutemyelogenous leukemia.? 2 ? In contrast, it displayed no significant inhibition for myelogenous leukemia cell lines THP1, U937 and KG-1, and the value of IC₅₀ were respectively 34.89?M?203.99?M? and 371.69?M at 48 hours. Meanwhile, When the concentration of linalool was 100uM, it showed little toxicity foe normal hematopoietic cells.2. Linalool induced obvious apoptosis of Molt-4 cells ?T-cell acute lymphoblastic leukemia?3. After treatment with linalool , the percentage of G₀/G₁, S, and G₂/M stage in Molt-4 cells declined notably, and the fraction of apoptosis cells increased obviously.4. Linalool could influence signal molecules in Molt-4 ?T-cell acute lymphoblastic leukemia? cells in 15 different human cancer pathways and represent chiefly that itup-regulated P53, P21, P27, P57 and GADD45a and so on.5. Results of Western Blot and RT-PCR revealed that after treatment with linalool, the expressions of P53, P21, P27, P57 and GADD45? were up-regulated and downstream proteins, c-jun and JNK, were activated.Conclusion1. Linalool could inhibit the growth of lymphoblastic leukemia cell lines, and induction of apoptosis is one of mechanisms for its suppression on the growth of these cell lines.2. Activating and up-regulating GADD45a/c-jun/JNK signaling pathway might be one of mechanisms for this apoptosis.3. Linalool could eliminate cells of quiescence effectively, suggesting that it maybe kill leukemia stem cells.

A Wildlife Vet, A Pigeon, A Groundbreaking Discovery

One of the reasons I love my job as a wildlife veterinarian so much is the variety of my days. No two days are the same; each is filled with different animals with different problems, and I love the anticipation of what my day may bring. I never know what kind of animal will come through the door, what challenges I?ll face, and what skills I?ll get to use ? it?s very unpredictable and that makes it so fun and exciting! My job is very fast-paced, keeps me on my toes, and there?s never a dull moment ? emergency medicine at its finest! It is also full of new discoveries, as I found out last spring.

Last April I was examining a wild male adult Rock Pigeon that couldn?t fly. The person who found the pigeon suspected a broken wing. On the initial physical exam, I didn?t find any broken bones but the bird was exhibiting odd neurologic behaviors, including ataxia (lack of coordination) and torticollis (head tilt).

My differential diagnoses included head trauma, aberrant parasite migration (a parasite that?s normally found in the gastro-intestinal system migrates to other parts of the body, like the central nervous system), and a viral or bacterial infection causing encephalitis (inflammation of the lining of the brain).

I started the bird on anti-inflammatories and moved it into our isolation ward in case he was potentially contagious to other birds. Over the next five to seven days, the bird?s neurological symptoms progressively worsened. He literally started standing on his head and spinning in circles (see video). The torticollis also worsened. Because of his decreasing status and poor prognosis I humanely euthanized him.

Normally, when we euthanize an animal or when an animal dies, it?s disposed of or saved under a salvage permit for a licensed organization to use in their collections, for research, etc. Sometimes though, we send animals out for additional testing to help pinpoint the cause of the animal?s illness or death. This is done via a necropsy (just like an autopsy but for animals).

Since Paramyxovirus, a highly contagious viral disease in pigeons that manifests as neurological symptoms, including ataxia and torticollis, was high on my list of possible diagnoses, I sent this bird to the University of Minnesota (UMN) Diagnostic Lab (D-Lab) where they would do a necropsy and test for the virus.

In the past WRC has had several pigeons showing similar signs and Paramyxovirus was always suspected. To my knowledge, however, none had ever been sent in for necropsy. A pattern was there, but what was it? What is going on with these neurologic pigeons, I wondered.

Well, the bird tested negative for Paramyxovirus. However, what the D-Lab did find was shocking. A parasite, named Sarcocystis calchasi, was found in cysts in the bird?s skeletal muscles. The really neat thing? This parasite had never been identified in the United States! It actually hadn?t been identified anywhere outside of Germany (where it was identified in 2009)!

When the pathologists contacted me to tell me this news, I was very excited (as were they!). Tissue samples were sent to Germany to confirm identification of the parasite, and sure enough it was S. calchasi.

The parasite is passed from bird to bird via infected feces, likely from the Northern Goshawk or Cooper?s hawk, which acts as final host in the lifestage of the parasite (does not cause symptoms in those birds). The parasite then migrates from the GI tract into the muscles of the bird. The aforementioned bird had a severe case of meningoencephalitis (inflammation in the brain); however, the parasite was not found there. The parasite?s role in causing neurologic symptoms is still unknown but the UMN?s pathologists are busy researching other birds.

With these amazing results, we promptly collaborated and published the findings in ?Transboundary and Emerging Diseases.? My first paper as a wildlife veterinarian has been published!

Dr. Arno Wunschmann, a veterinary diagnostician at UMN who discovered the S. calchasi cysts, speaks of what impact this discovery has on the global transmission of diseases:

?This case demonstrates that diseases can spread very quickly and how the use of modern diagnostic tools can help diagnose diseases (in this case PCR). The disease was first described in 2009 in Germany and showed up 2 years later already in Minnesota.

?I was only to able to put the pieces together because somebody in Germany had worked hard to get his cases published in a timely fashion (because the first cases were only seen two years before the paper was in print) and I used the literature resources (including the internet). It is a beautiful example of how the scientific community as a whole is meant to function and can function. Alternatively, one can speculate that the disease went undiagnosed for some period of time but that seems unlikely because the lesion is very obvious (if a pathologist examines the muscle).?

S. calchasi is now recognized as an important differential diagnosis in neurologic pigeons.

How many pigeons possibly have it and have been misdiagnosed? That?s hard to say, but since S. calchasi was discovered, we?ve submitted every pigeon (all wild birds, not domestic) that dies while in our care or is euthanized. Nearly 20% have tested positive for this parasitic infection. This is huge news in the tracking of wildlife diseases on a global level.

One of the best parts of this discovery? My curiosity paid off! Wunschmann states: ?The case demonstrates nicely how important it is that curious clinicians do not just accept the status quo and submit animals for necropsy. Compliments to you!?
